Subject: Embassy Walk-In Hours to Assist with Completion of Form DS-5528 Evacuee Manifest and Promissory Note
The Department of State has arranged a special chartered flight for U.S. citizens and U.S. Legal Permanent Residents (LPRs) currently expected to depart Vientiane around 5:00 p.m. on April 11 for San Francisco, CA. Exact departure time and routing are subject to change.
Every person requesting a seat on this chartered flight must complete a Form DS-5528 Evacuee Manifest and Promissory Note. We encourage all individuals who have previously conveyed their interest in this flight by completing our survey to complete Form DS-5528 and submit it electronically. If we do not have your information in our survey and have not received a completed Form DS-5528 Evacuee Manifest and Promissory Note from you confirming your interest in this flight, we will not make a seat available for you.

U.S. Citizens who cannot complete the Form DS-5528 and survey electronically or have questions about the Form DS-5528 or the chartered flight are asked to come to the Embassy on Wednesday, April 8 and Thursday, April 9 between 1:00 p.m. and 4:00 p.m. English, Lao, and Hmong speaking staff will be on hand to answer questions and help you complete forms.
Please note that seats on this flight are limited. U.S. citizens and LPRs who have already completed our survey and confirmed interest to laosevac@state.gov should complete the Form DS-5528 Evacuee Manifest and Promissory Note immediately. If you cannot complete any of these steps electronically but would like a seat on the chartered flight, you must come to the Embassy on Wednesday, April 8 or Thursday, April 9 between 1:00 p.m. and 4:00 p.m. to complete forms and establish your eligibility for evacuation. English, Lao, and Hmong speaking staff will be on hand to answer questions and help you complete forms.
Completing the Form DS-5528 Evacuee Manifest and Promissory Note is your formal agreement to reimburse the U.S. Government for the flight. Current estimated cost for the flight is $1900/adult. Costs for children and infants are lower. No cash or credit card payments will be accepted. You will be responsible for any arrangements or costs (lodging, onward destination or local transportation, etc.) beyond your initial destination in the United States.

U.S. citizens without a valid U.S. passport who wish to depart on this flight should send their name, date of birth, and the number of their most recent expired passport to laosevac@state.gov. Please indicate that the passport is expired. You will be contacted with information on how to obtain an emergency passport.
U.S. Legal Permanent Residents (holders of valid Green Cards) who wish to depart on this flight should send their name, date of birth, and A number (Green Card number) to laosevac@state.gov.
Unless you receive an email confirming your flight, please do not come to the airport. Only those with email confirmations will be permitted to fly.
The Embassy is not aware of any option available to U.S. Citizens for leaving Laos via commercial means. U.S. citizens who wish to return to the United States should avail themselves of this chartered flight unless they are prepared to remain in Laos indefinitely.
