The followings are required:

  1. Make an appointment here
  2. Complete Passport Application form DS-82 online but do not submit it electronically. You may obtain a blank copy of the form in our office on your appointment date
  3. Your most recent U.S. Passport
  4. One recent photo. Click here to see passport photo example
  5. Your social security number, if issued
  6. A fee of $130, payable in cash (LAK or USD) or by credit card

OPTIONAL: If your name has changed, please provide an original or certified marriage certificate or court order.  It is recommended that at least one parent appear in person with children ages 16 and 17.
